Essential Health is proud to start offering Sports Physics For students-athletes, from college to college, in Northern Dakota, Minnesota and Wisconsin, which will be valid for the next school year and beyond.
In northern Dakota, sports physics led on April 15 or after are good for the next school year. In Minnesota, sports physics completed on April 13 or after are good for three years, and in Wisconsin, they are valid for two years when finished on April 1 or after.
A sporting physique, also known as the Physical Préparticipation (EPI) examination, helps to determine whether it is sure for your child to participate in a certain sport. Most states, including Northern Dakota, Minnesota and Wisconsin, demand that athlete students have a sports physique before they can start a new sport (including all practices) or start a new competition season. It is recommended that your child has his physique at least six weeks before the start of his sports season, so there is enough time to follow something if necessary.
The forms are available online at ndhsaa.com (North Dakota), mshsl.org (Minnesota) and wiaawi.org (Wisconsin). Please complete and sign the appropriate form before your appointment and bring a copy when you come for your child’s physique. This form can also be completed via MyChart with an active account.
“A sports physique can help you determine if your child has health problems that could interfere with their sports or increase their chances of injury during competition,” said Dr Tanya DiegelPresident of primary care division in Essentia. “But this is not limited to organized sports. Many children participate in physically demanding activities and hobbies, while others are not as physically active. Anyway, all children should receive a physique of their care provider.”
Sports physics can be completed alone or as part of a Well-child / adolescent visit. Good children / adolescent visits are the ideal time to review your child’s health in all areas – growth, cognitive and social development, disease prevention, mental health and more. These visits are recommended each year.
You can plan these visits directly through your pediatrician or family medicine supplier. You can also plan by calling (844) 663-1068, online via Essentialth.org or through your ESSENTIA MYCHART account.
